The fourth cranial nerve, known as the trochlear nerve, is a motor nerve that innervates the superior oblique muscle of the eye. Fourth Cranial Nerve palsy is a condition in which the superior oblique muscle becomes paralysed. The common cause of Fourth Cranial Nerve Palsy is traumatic head injury. Fourth Cranial Nerve Palsy can be both congenital and acquired. This condition can cause vision concerns as the eyes are misaligned. Double vision may be present, which can be vertical, diagonal, or even torsional. People with Fourth Cranial Nerve Palsy usually compensate by tilting their head, as this can help in aligning their eyes better. The initial treatment of Fourth Cranial Nerve Palsy is through prism glasses that correct the vision. When prism glasses do not work, eye muscle surgery is performed.
